Position may supervise other administrative and clerical support staff for a department or departments. Responsibilities
- Handle and coordinate active calendars
- Maintains department personnel paperwork
- Prepares and distributes monthly reports
- Travel as needed to area facility sites
- Schedule and confirm meetings
- Ensure file organization based on office protocol
- Provide ad hoc support around office as needed
Qualifications
- Bachelor’s degree or equivalent experience

Networks Connect is hiring for an Ambulatory Nurse Case Manager located in Washington D.C. In this position you will comprehensively and actively maintain the care management and coordination needs of payer defined and/or payer enrolled populations of patients who meet designated inclusion criteria. This includes activities such as assessments, formal Case Management care plan development, establishing goals and intervention and monitoring/tracking. Through these activities, the Ambulatory Nurse Case Manager will partner with physician practices and the care management team related to the clinical and care coordination needs of the patient, as well as work with payers and community resources to develop and facilitate effective, efficient care delivery options for the patient across the continuum of care.
Qualifications
Minimum Education
BSN (Required)
Master’s Degree (Preferred)
Minimum Work Experience
4 years Nursing experience in case management, ambulatory nursing or community/homecare experience (Required)
Required Skills/Knowledge
Ability to critically think and apply logic and reasoning to dynamically changing healthcare environment.
Requires superior verbal communication skills and service excellence approach with internal and external stakeholders.
Must have strong business writing skills.
Proficient at keyboarding and facile with Microsoft Office Excel, Access and Power Point.

JOB DESCRIPTION
The medical receptionist will be the first point of contact so they will need to make a good first impression. We want someone on the front lines who has a professional appearance and attitude. Duties include offering administrative support across the organization. They will welcome guests and greet people who visit the clinic. They will also coordinate front-desk activities, including verifying patients as well as collecting insurance information.
To be successful as a Medical Receptionist, they should also be able to deal with emergencies in a timely and effective manner, while streamlining office operations. Multitasking and stress management skills are essential for this position.
1. Receiving visitors at the front desk by greeting, welcoming, directing and announcing them appropriately. Educate and assists patients on signing into Clockwise MD. Educate and provide information to the patient regarding the patient portal.
2. Provide basic and accurate information in-person and via phone/email. Very knowledgeable on HIPAA regulations and maintains confidentiality. Maintains annual and additional training on HIPAA regulations and compliance.
3. Prepares an order for the front office supplies and keeps inventory of stock. Works closely with the manager to determine appropriate levels of supplies. Turns in the order for manager to place.
4. Have patients fill out information sheets such as patient demographics, workers comp, drug screen, occupational med or Epworth sleep scale forms. Accurately enters patients, their demographics and their insurance into the EMR system. Verifying all demographic information and insurance information with each visit. Utilize the EMR system to verify real time insurance information.
5. Collecting payments and post them to the correct accounts. This includes collecting co-pays prior to the patient being seen and collecting balances for services and/or supplies before the patient leaves. Maintains an accurate funds log for all money received and posted by the receptionist for the day. Count the cash drawer at the beginning and end of each shift and initial that this procedure was done. Maintain an accurate funds log of the money in the cash drawer. Have a clear understanding of the payment process within the EMR to include “save/finalize”, running a credit card, and issuing a credit back to a credit card. Writes a hand receipt for all CASH received.
6. Call other medical facilities, companies, insurances, etc. for additional information needed to treat the patient. Ensures appropriate release of information or occupational medicine release forms have been filled out by the patient.
7. Educate patients on membership plans. If a patient would like to sign up as a member, help facilitate that process though our website portal. Answer all questions regarding the membership that the patient may have. Instruct the patient to review the membership rules and point out that it is a 12-month contract for consumers (white cards).
8. Make copies of sports physical papers, workers comp restriction/release forms, other important documents for patients, referring providers and/or employers. Manages all record requests by securing a release, gathering the requested information, securing payment if it is someone other then the patient or referred provider, and mailing or faxing to the appropriate requestor. Understands medical record requests process and how it corresponds to HIPAA.
9. Reports to work on time and unlocks the front door and opens the clinic. Maintains the cleanliness of the reception room throughout the shift. Makes sure the reception bathroom is clean and in working order.
10. Makes sure that all forms used for gathering patient data, patient information, and patient handouts on policies or procedures are up to date and accurate. Maintains a file for these forms that can be readily handed out if requested by the patient. These files referred to on our demographic sheet include our Arbitration Policy, Financial Policy, HIPAA form, Outside Service Agreement and Membership Cost Breakdown.
11. Understands the process of checking in a patient who has a membership. Fully understands how to find the patient within the HINT platform prior to checking the patient in. Has the “membership cost breakdown” policy readily available for any questions the patient may have regarding their personal responsibility for the costs incurred for the visit. Understands the procedure if the patient is not on the HINT platform but insists they are a member. Collects all membership discounted fee prior to the patient being seen for a white card member and collects the discounted fee that is due for services and/or supplies prior to the patient leaving.
12. Will maintain education requirement with the EMR system by completing the FRONT DESK STAFF TRAINING. A validation form will be downloaded, the training video watched, the supplemental materials done along with the deep dive topics. This will be done every 6 months and the validation form will be sent to human resources to be stored in the employee file.
13. Performs other job duties that may be assigned from time to time.

Networks Connect Healthcare Staffing is currently conducting a search for full-time and part-time Nurse Practitioner – NP for our home health client. In this role, the nurse practitioner will conduct in-home and at times telehealth health assessments serving a primarily geriatric patient population. The nurse practitioner will work with physicians, advance practice providers and other members of a clinical team to identify high risk patients and develop a care delivery pathway that addresses patients’ physical, psychological, and environmental needs. Assess health risk behaviors, and work within the context of a multidisciplinary approach to care to provide quality and sustainable services. Employ advance level critical thinking and superior skill sets to deliver care and improve lives of older disabled and often vulnerable patients. Consistently demonstrates clinical competency in the management of chronic diseases for patients with complex comorbidities. Work with patients and families to improve their quality of life, reduce health risk factors and mitigate circumstances. Implement clinical pathways and adhere to best practices that yield compliance and improve outcome for each patient. Educate patients and families and support them in assuming responsibility for their health by reducing further exacerbation of their disease, improve their overall health status and attain their goals.
Communicate, consult, and collaborate with physician partners in the delivery of quality patient care protocols and practices; with the aim of reducing hospital readmissions, ED utilization, and closing care gaps. Be a strong patient advocate and work to minimize fragmented care within the care delivery system, to ensure all services provided to patients are seamless and efficient.
The territory assignment of the traveling nurse practitioner performing house calls will be defined based on current business needs.
Requirements:
- Travel up to 100%
- Functions to the full level of the NP/PA license as a member of a multidisciplinary care team to provide age-appropriate, high-quality health care as appropriate to the patients receiving clinical management services
- Performs home visits as required. Completes full assessment of patient including physical exam and evaluation of all medical conditions, diagnosing, assessment of the home environment, medication review, educate the patient, and identify needed referrals. Addresses any issues identified during the course of the visit
- Performs telehealth visits as needed to include assessment of medical conditions
- Differentiates normal aging from illness and disease processes and uses standardized assessment processes to assess social support and health statuses such as cognition, mobility, pain, skin integrity, quality of life, nutrition, neglect, and abuse
- Work closely with the care team, PCP’s, and families to assess risk and implement individualized interventions to minimize the risk and develop strategies to reduce utilization and hospital readmission
- Identify need and frequency of visits based on patient’s health status and articulate need to social work /nurse Health Coaches as deemed appropriate.
- Understand EHR system and document all patient contacts in a timely, accurate and structured manner.
- Demonstrates accuracy in billing as evidenced by successful completion of annual billing and coding modules and internal compliance audits
- Works with the team to track data and develop quality metrics to ensure efficacy and improve outcomes
- Demonstrates cultural competence in all interactions with patients, caregivers, and other members of the team. Provides care that is respectful of and responsive to cultural and linguistic needs.
- Meets department productivity and quality goals. Identifies and reports any barriers to achieving these goals
Registration, Licensure, and/or Certification:
Education and Training: Current licensure to practice as a Certified Registered Nursing Practitioner (CRNP), FNP, GNP, ANP, DNP, or Physician Assistant who is board certified or eligible with an active, unencumbered licensure free any open / unresolved disciplinary actions including probation or restrictions against privilege to practice in the Commonwealth of Pennsylvania
Experience:
- 3+ years of clinical experience as a Nurse Practitioner in related clinical field
- Experience in geriatrics is desired
- Experience in home health care or nursing facilities is desired
