Networks Connect is seeking a Acute Care Case Manager that will work in a hybrid setting. We are hiring an Acute Care Case Manager (Hybrid) to provide exceptional case management services, ensuring seamless transitions of care for our patients and their families.
Position Overview
The Acute Care Case Manager will coordinate comprehensive case management services, including assessment, care planning, intervention, and follow-up to ensure optimal patient outcomes. This role involves collaboration with multidisciplinary teams, administrative support, and community outreach.
Key Responsibilities:
Patient Case Management & Care Coordination
- Conduct initial assessments to identify patient and family needs for discharge planning.
- Develop and implement care plans that align with healthcare team goals.
- Facilitate care transitions by connecting patients with appropriate external providers and resources.
- Communicate changes in care levels, high-risk factors, and insurance denials to reduce financial risks for patients and the hospital.
- Provide clinical reviews and document authorizations/denials from third-party payers.
Collaboration & Community Outreach
- Partner with internal departments and external organizations to enhance patient care.
- Engage in community outreach initiatives to support pediatric healthcare services.
- Advocate for patient-centered care and ensure families receive necessary resources.
Performance Improvement & Compliance
- Identify opportunities to improve case management processes.
- Ensure compliance with all legal, regulatory, and hospital policies.
- Maintain detailed and accurate documentation in accordance with department protocols.
Qualifications:
Education:
- Bachelor’s Degree in Nursing (Required)
- Master’s Degree (Preferred)
Experience:
- Minimum 5 years of nursing experience, including at least 3 years in a pediatric setting (Required)
Licensure & Certifications:
- Active Registered Nurse (RN) License in Washington, D.C. (Required)
- Certification in Case Management or Utilization Management (Preferred)
Additional Information:
- Schedule: Full-time, 10-hour shifts, every other weekend + 2 weekdays
- Work Location: Washington, D.C.
- Salary Range: $85,000.00 – $125,000 (based on experience)
